PREPARING FOR YOUR FIRST VISIT I am sincerely looking forward to our first meeting. To ensure that you get the most from our time together, please take time now to print the following Forms, complete the appropriate information and bring them with you to your first appointment. If you would like to schedule please call me at 303-753-9258. I return most phone calls within an hour or two. This Form includes the Psychotherapy Disclosure Agreement which has information required by State Law which you need to read, understand, sign and bring to your first session. Insurance can be discussed during the first visit. Please complete and bring with you the Intake Form that has relevant information and bring your insurance card and insurance authorization to your first appointment. The Information Release Form allows release of your confidential information whenever needed to help with reimbursement and you restore your position in the community. A signed release form is required prior to any release of ANY information to a third party such as a previous or current health care provider, school, lawyer or other interested person. What Can I Expect at My First Visit? You can expect to be greeted on time and invited in my office where we will spend the first ten minutes discussing policies, procedures, and financial arrangements. I will then ask you to tell me the major reason you are seeking counseling/therapy. I will ask what the origin of the issue is and what you have tried to resolve this problem. I will ask some medical questions regarding medications you are taking, your sleeping and eating pattern and your relationships with your family and friends. After about 40 minutes I will attempt to give some feedback and some initial strategies we may want to consider in the future. Unlike the movies you do not lie on a coach and discuss your potty training or other irrelevant issues. You will be told when your time is up and another appointment will be made and you will pay your co-pay or payment in full if no insurance. When you leave the office, you will most likely feel relieved and a sense of hope for a better life returns. You will most likely look forward to your next visit.
